Events on the mountain are documented using pre-printed forms. This page lists the most common forms patrol members will use and are provided to become better familiar with during training. When performing Base Patrol Equipment inventory and there is a shortage of forms, use the Master Binder located in the locker room to make more copies and file. All forms are stored in the accordion folder on the desk in the aid room.
Patrol Morning Briefing. Form used by the Patrol Manager to brief patrollers each morning.

Daily Roster/Opening Assignments/Daily Closing Log. Identifies patrollers performing duty days and their responsibilities for opening and closing routes and areas on the mountain. The Patrol Manager assigns patrollers to their duty stations and openers in the morning brief and the Dispatcher assigns closers in the afternoon.

Daily Dispatch Log. Form used to record everything that happens on the mountain, (e.g. medical response, SAR, lift down, courtesy ride, avalanche control, reports of wild animals on the mountain, etc.)

Incident Report Forms. Forms used to record medical care provided. Fill form out completely, using NA for sections not applicable. Several of these forms may be required for the same incident depending on location and type of illness or injury.

Rental Return Form. Used when a guest is injured when they have rented equipment and taken with equipment to Rental Shop for inspection.

Fax Cover Sheet. Form used as a cover sheet to transmit forms to Beartooth Billings Clinic. After forms are faxed, place them face down in right hand drawer.

“Bad Boy” Logbook. Logbook maintained at Dispatch with sections divided alphabetically. Guests who violate mountain policy are counseled on their infraction and their name is recorded by their last name in the logbook. Guests may lose their pass.
